A KFF analysis finds that over half the country's hospitals will be penalized under CMS's patently flawed hospital readmissions reduction policy, which the agency has stubbornly refused to fix or enough acknowledge is very, very poorly designed.  Most of the hospitals that won't be penalized are exempt from being evaluated.
